ABHA & ABDM

ABDM Explained: How the Ayushman Bharat Digital Mission Is Transforming Healthcare in India

India's healthcare system has historically been fragmented — a patient's X-ray sits in one hospital's file room, their blood test report in another clinic's register, and their prescription history nowhere at all. The Ayushman Bharat Digital Mission (ABDM), launched by the National Health Authority (NHA), is the government's answer to this problem: a unified, consent-based digital health ecosystem that connects patients, doctors, hospitals and diagnostic centres on a common digital rail.

The Core Building Blocks of ABDM

1. ABHA — Health ID for Patients

Every patient can create a free ABHA (Ayushman Bharat Health Account) — a 14-digit health identity that links their medical records across providers. Read our detailed guide on creating your ABHA ID to get started.

2. HPR — Healthcare Professionals Registry

Doctors and other healthcare professionals register on the Healthcare Professionals Registry (HPR), receiving a verified HPR ID after Aadhaar-based identity and qualification checks. This builds a trusted, verifiable national directory of doctors — patients can be confident that a doctor consulting them digitally is genuinely licensed and verified. On Rejuvenate Digital Health, doctors complete HPR verification directly from their profile using an Aadhaar OTP flow that checks their claimed HPR ID against NHA's records.

3. HFR — Health Facility Registry

Hospitals, clinics, labs and pharmacies register under the Health Facility Registry (HFR), creating a verified national directory of health facilities — an essential layer for patients to trust the providers they interact with digitally.

4. Consent-Based Health Data Exchange

ABDM does not create a central medical database. Instead, records stay with the provider (hospital, clinic, lab) that generated them. When a patient wants to share their history with a new doctor, a Consent Manager mediates the request — the patient approves exactly what is shared, with whom, and for how long. This is what makes ABDM fundamentally different from a centralised health records system: patients remain in control.

How Care-Context Linking Works

When a doctor on an ABDM-enabled platform finalises a prescription for an ABHA-linked patient, that consultation can be registered as a care context and linked to the patient's ABHA. This means the next time the patient visits any other ABDM-enabled facility and grants consent, that doctor can see the earlier prescription, diagnosis and advice — without the patient having to remember or repeat their medical history. Rejuvenate Digital Health supports this HIP-initiated (Health Information Provider) linking as part of our ABDM integration roadmap, so finalised e-prescriptions for ABHA-linked patients are queued for linking automatically.

Why This Matters for You

  • Patients get portable, lifelong health records and faster registration at any ABDM-enabled facility.
  • Doctors get verified digital identities and, over time, faster access to a patient's relevant history (with consent), improving diagnosis and reducing duplicate testing.
  • Hospitals and clinics get interoperability — the ability to exchange records safely and standardly with any other participating facility in India.

Where ABDM Is Headed

ABDM is still evolving. Consent artefact management and Health Information User (HIU) workflows — where a doctor formally requests a patient's past records through the consent framework — are the next frontier for most platforms, including ours. As these capabilities mature, digital health platforms like Rejuvenate Digital Health will be able to offer patients an even more complete, portable and secure health record — one that travels with them for life, not just for one hospital visit.

Whether you're a patient creating your first ABHA ID or a doctor completing HPR verification, every step you take today builds towards a healthcare system where your medical history is always available when and where you need it — safely, and only with your consent.
